It seems like the USB 3.0 controller is not enabled in the BIOS. Perhaps the the BIOS has a bug in it, although I would think that Asrock would be aware of it by now. Perhaps flashing to one or two versions older may help. You can always flash back if it doesn't.

When I received the first new board I immediately flashed it from 1.4 to 1.7 so I never had a chance to see if the 3.0 ports worked with the old bios. When I installed the new replacement board I intentionally waited to flash the bios but the 3.0 ports still did not work with the older 1.4 bios.

Have you tried UEFI mode?

Have you tried using the 2nd 3.0 header? Looking at the package contents it does not list a rear panel adapter for it, so that may not be an option if you don't have a spare lying around.

Physically there are two rear panel USB 3.0 (blue) ports, the Motherboard Layout and I/O Panel on page 2 and 3 of the manual show them, the specifications on page 7 state "-2 x Ready-to-Use USB 3.0 Ports", and the bios has an option to enable/disable the USB 3.0 ports so I have to assume the Z77M is indeed equipped with them.

My Lian-Li case is three years old and only has front/top panel 2.0 ports so I have not yet been able to try the 3.0 header. However, I just ordered a 2.5" drive bay panel with two 3.0 ports on it. I expect it in a few days and am looking forward to see if the header works. If it does work it will fully serve my needs so I will then give up on the two rear 3.0 ports as they will not be needed.
